Home and Community-Based Services (HCS) are person-centered care options that provide medical and social support to individuals in their homes or communities, rather than in institutional settings. These services are designed for people with functional limitations, such as older adults and individuals with disabilities, and can include assistance with daily living activities, home health care, and therapies. HCS programs, funded through programs like Medicaid, allow people to maintain their independence and quality of life.
What they are
Person-centered care: HCS programs focus on the individual’s needs and allow them to continue living in their community.
A range of services: Programs can provide a combination of medical and non-medical services, including personal care, homemaker services, and therapies.
Support for various groups: HCS serve diverse populations, including older adults, people with intellectual or developmental disabilities, and those with mental health or substance use disorders.
